Dr. Kaykha is responsible for providing you with the finest endodontic care anywhere. You are responsible for compensating us for your care. It is the responsibility of our administrative staff to arrange treatment compensation.

We do not accept any HMOs, public aid plans, medicare or medicade plans.
At the time of your consultation we will contact your dental insurance company for the benefit information and will estimate your insurance co-pay as quoted by your insurance company. Your co-pay (out-of-pocket expense) will be due at the time of service. If you have a balance on your account after the insurance company has made a payment, you will be responsible for the remaining difference.
If you are enrolled in a dental plan we are not contracted with:
Payment of the total fee will be required by the completion of treatment. We will submit or file the necessary forms to your insurance company on your behalf. Care Credit is also available for those interested in no-interest financing options.
For patients without dental insurance:
If the treatment is completed in one visit, 100% of the total fee will be due at the time of service. If treatment is completed in multiple visits, 50% of the total fee will be due at the beginning of treatment and the remaining 50% will be due at the time the treatment is completed.
If you have questions about financing, please consult the front desk manager before any treatment is started. Financing is available for qualified patients through Care Credit, which includes interest-free monthly payment plans.
